The infection due to M. intracellulare can manifest itself as a fibrocavitary lung disease, as a hypersensitivity pneumonitis, or as a bronchiectasis superinfection, one of which variants is the Lady Windermere syndrome (LWS), affecting slender elderly women, with higher incidence of scoliosis, and with bronchiectasis and/or nodules in the middle lobe or lingula. A case of a 61-year-old female patient is reported –with a diagnosis of COPD, extreme thinness, and scoliosis–, who consults due to a respiratory syndrome of cough with expectoration, general malaise, fever and tiredness. The tomography shows evidence of bronchiectasis and nodules in the middle lobe and lingula. A sputum culture is ordered for mycobacteria and bacilloscopy. The results were ZN ++++ and culture positive. A molecular identification was conducted at the Instituto Malbrán. Isolation showed a 100% identity with the pattern strain of Mycobacterium intracellulare ATCC 13950. Knowing whether a lung disease caused by environmental mycobacteria –such as LWS– exists would allow an earlier diagnosis, avoiding the clinical deterioration of the patient, and a lower charge on the resources of medical attention during long periods of time. Micobacterium intracellulare, es un bacilo ácido alcohol resistente, no tuberculoso, que forma parte del complejo Mycobacterium avium. La infección por M. intracellulare puede manifestarse como una enfermedad pulmonar fibrocavitaria, una neumonitis por hipersensibilidad, o una sobreinfección de bronquiectasias, una de cuyas variedades es el Síndrome de Lady Windermere (SLW), éste afecta a mujeres delgadas, de edad avanzada, con mayor incidencia de escoliosis, y presencia de bronquiectasias y/o nódulos en lóbulo medio o língula. Se reporta un caso de paciente femenino de 61 años, con diagnóstico de Enfermedad Pulmonar Obstructiva Crónica (EPOC), extrema delgadez, escoliosis, que recurre a consulta por cuadro respiratorio de tos con expectoración, malestar general, fiebre y decaimiento, la tomografía evidencia bronquiectasias y nódulos en lóbulo medio y língula. Se solicita cultivo de esputo para micobacterias y baciloscopía. Los resultados fueron tinción de Ziehl Neelsen (ZN) ++++ y cultivo positivo. Se realizó identificación molecular en el Instituto Malbrán. El aislamiento presentó 100% de identidad con la cepa patrón Micobacterium intracellulare ATCC 13950. El conocer la existencia de la enfermedad pulmonar producida por micobacterias ambientales, como el SLW, haría posible llegar al diagnóstico en menor tiempo, evitando el deterioro clínico del paciente, y una menor carga en los recursos de atención médica durante largos períodos de tiempo.
