Petroli, F. bacteremia septic shock pediatrics invasive infection pneumonia bacteriemia shock séptico pediatría neumonía Streptococcus pyogenes is the main bacterial pathogen isolated in acute pharyngitis, occasionally it is the causal agent of pneumonia and bacteremia, generally associated with viral infections (influenza, measles). A 5-year-old girl with respiratory distress is admitted to the pediatric ward. Pneumonia was verified by radiographic plate, he entered respiratory mechanical assistance and antibiotic treatment was started. Two hours later, he presented symptoms of septic shock, generalized coagulopathy, and multiple organ failure with subsequent death. Blood cultures, CSF, and post-mortem nasopharyngeal aspirate are taken and sent to the laboratory. The blood cultures are positive and show Gram-positive Gram cocci staining and the biochemical tests: Pyr (+), Bc (+), serological tests point to group A beta hemolytic Streptococcus. The CSF was negative. The nasopharyngeal aspirate showed Influenza B as a result. Streptococcus pyogenes can produce very serious conditions, complicating and culminating in a septic shock caused by the toxin of the microorganism whose course is fulminant. El Streptococcus pyogenes es el principal patógeno bacteriano aislado en las faringitis agudas, ocasionalmente es el agente causal de neumonía y bacteriemia generalmente asociado a infecciones virales (influenza, sarampión). Ingresa a la sala de pediatría una niña de 5 de años con dificultad respiratoria. Por medio de placa radiográfica se constata neumonía, entra en asistencia mecánica respiratoria y se comienza tratamiento antibiótico. A las dos horas presenta cuadro de shock séptico, coagulopatía generalizada y fallo multiorgánico con su consiguiente fallecimiento. Se le toman hemocultivos, líquido cefalorraquídeo (LCR) y aspirado nasofaríngeo post morten y se envían al laboratorio. Los hemocultivos se positivizan y dan en la coloración de Gram cocos Grampositivos y las pruebas bioquímicas: Pyr (+), Bc (+), pruebas serológicas orientan hacia Streptococcus beta hemolítico grupo A. El LCR fue negativo. El aspirado nasofaríngeo arrojo como resultado Influenza B. El Streptococcus pyogenes puede producir cuadros muy severos complicando y culminando con un shock séptico causado por la toxina del microorganismo cuyo curso es fulminante.
