Cell morphology was estimated by light microscope; Rigidity index by filtration; Osmotic fragility (measured photometrically at 540 nm), obtaining X50 value (NaCl mM concentration yielding 50% hemolysis) and β (degree of homogeneity of the cell population). Erythrocyte aggregation was measured by optical densitometry showing S, mean size of red cells aggregates and V, initial velocity of process. There was no alteration of erythrocyte aggregation neither in S nor in V. Presence of methotrexat in incubation medium increased the osmotic fragility (higher X50) as well as heterogeneity of the population response to hyposmolarity in the 1 and 5 µM doses. The former effects added to stomatocytosis and erythrocyte deformability could be related with the anemia in patients under Methotrexat treatment. In erythrocyte aggregation, although the size and speed of the aggregates do not increase in presence of the drug, it would be expected that when the rigid erythrocytes enters the capillaries could give time to the formation of aggregates that difficult circulation. and promote the destruction of red blood cells.

El metotrexato es uno de los fármacos que generalmente se emplea en el tratamiento del cáncer y de la artritis reumatoide. Entre las reacciones adversas encontradas está la anemia, por lo que nos propusimos estudiar el efecto directo del metotrexato sobre la reología eritrocitaria in vitro, utilizando muestras de donantes de sangre normales que se dividieron en 5 alícuotas: una se utilizó como control y a las otras se les adicionó metotrexato en concentraciones crecientes (n:5, cada grupo). La forma celular fue estimada con un microscopio óptico; el índice de rigidez por filtración; la fragilidad osmótica (medida fotométricamente a 540 nm), obteniendo valor X50 (concentración de NaCl mM con 50% de hemólisis) y β (grado de homogeneidad de la población celular). La agregación eritrocitaria medida por densitometría óptica ha estimado S, tamaño promedio de los agregados eritrocitarios y V, velocidad inicial del proceso. La presencia de metotrexato en el medio de incubación aumentó la fragilidad osmótica (mayor X50) e incrementó la heterogeneidad de la respuesta poblacional a la hiposmolaridad con las dosis de 1 y 5 µM. Estos efectos sumados a la estomatocitosis y menor deformabilidad pueden relacionarse con la anemia encontrada en los pacientes tratados con metotrexato. En la agregación eritrocitaria, si bien el tamaño y la velocidad de los agregados no se incrementan en presencia del fármaco, sería esperable que la dificultad de los eritrocitos con rigidez al ingresar en los capilares pueda dar tiempo a la formación de agregados que dificulten la circulación y favorezcan la destrucción de los glóbulos rojos.
