Based on a situated experience in a transitional, open-door residential device intended for people with severe mental health conditions, the analysis examines interventions oriented toward the recovery of autonomy, the reconstruction of social bonds, and social and labor inclusion. The analysis focuses on the discharge group as a key space for problematizing the subjective and social effects of prolonged hospitalizations, articulating the losses and the marks left by institutionalization in users’ lives with the strategies implemented in the process of psychosocial rehabilitation. Within this framework, three central axes of the rehabilitation process are developed: housing, social networks, and work, identifying in each of them the main obstacles and facilitating factors for discharge and community reintegration. The findings show that deinstitutionalization goes beyond the physical closure of psychiatric hospitals and requires intersectoral public policies and sustained institutional commitments over time. El presente trabajo tiene como objetivo reflexionar en torno a las estrategias de rehabilitación para la desinstitucionalización en el marco del grupo de egreso de una residencia protegida de salud mental de la Ciudad Autónoma de Buenos Aires, focalizando en los limitantes y posibilitadores que emergen en dicho proceso. A partir de una experiencia situada en un dispositivo habitacional transitorio y de puertas abiertas, destinado a personas con padecimientos mentales severos, se analizan intervenciones orientadas a la recuperación de la autonomía, la reconstrucción de vínculos y la inclusión social y laboral. El análisis se centra en el grupo de egreso como espacio privilegiado para problematizar los efectos subjetivos y sociales de las internaciones prolongadas, articulando las pérdidas y las marcas que imprime la institucionalización en la vida de les usuaries con las estrategias desplegadas en el proceso de rehabilitación psicosocial. En este marco, se desarrollan tres ejes centrales del proceso de rehabilitación: hábitat, red social y trabajo, identificando en cada uno de ellos los principales obstáculos y facilitadores del egreso y la reinserción comunitaria. Se evidencia que la desinstitucionalización excede el cierre físico de los hospitales psiquiátricos y requiere políticas públicas intersectoriales y compromisos institucionales sostenidos en el tiempo.
