Through interviews with professionals from primary health care services, the gaps between what health protocols propose and the actual processes of detection and care for women in situations of gender-based violence are described, highlighting tensions between the logic of reporting and the logic of care. The notions of victim and agency in the care processes in the health system are investigated, reflecting on the possibilities and obstacles contained in the reporting processes for gender-based violence, and the tensions with health processes. El presente artículo aborda el mecanismo de denuncia en situaciones de violencia basada en género, examinando su abordaje legal, los obstáculos del sistema judicial, y el rol del personal de salud como auxiliares de dicho sistema. A partir de entrevistas a profesionales de centros de atención primaria de la salud (CAPS) de Morón, provincia de Buenos Aires, realizadas entre 2016 y 2019, se describen las brechas entre lo que plantean los protocolos sanitarios y los procesos reales de detección y atención a mujeres en situación de violencia basada en género, lo que evidencia las tensiones entre la lógica de la denuncia y la lógica del cuidado. Se indaga en las nociones de víctima y agencia en los procesos de atención en el sistema de salud, reflexionando sobre la posibilidades y obstáculos que contienen los procesos de denuncia por violencia basada en género, y las tensiones con los procesos de salud.
