There is enough evidence to support that during the first psychotic episode there are important neurobiological and psychosocial changes, which considerably determine the future prognosis, especially as regards the overall performance and the levels of disability (“critical period hypothesis”). In turn, there is evidence supporting that less untreated psychosis time during the first period is associated with a better prognosis. This has lead to the development of interventions to allow for detection as early as possible from the onset of the first psychosis symptoms. The results obtained (all in developed countries) show a marked improvement in the untreated psychosis time, in recovery and prognosis of the disease and in reduction of associated risks (such as drug abuse and suicide). Based on these antecedents, the WHO has recommended that the countries should adopt policies on the issue, focused on early detection in individuals with psychotic episodes. Se realiza una revisión de literatura acerca de los fundamentos y la evidencia que permitiría sustentar una intervención de detección temprana para el primer episodio de psicosis. Existe suficiente evidencia que permite sustentar que durante el primer episodio de psicosis se producen cambios importantes a nivel neurobiológico y psicosocial, que determinan en forma sustancial el pronóstico futuro, especialmente en lo que se refiere al grado de funcionamiento global y a los niveles de discapacidad (“hipótesis del período crítico”). A su vez, hay evidencia que demuestra que un menor tiempo de psicosis no tratada durante el primer episodio se asocia con un mejor pronóstico. Esto ha motivado el desarrollo de intervenciones que permitan una detección lo más temprana posible desde el inicio de los primeros síntomas de la psicosis. Los resultados obtenidos – todos en países desarrollados - muestran una notable mejoría en el tiempo de psicosis no tratada, en la recuperación y pronóstico de la enfermedad, así como en la reducción de riesgos asociados (tales como el consumo de drogas y el suicidio). Estos antecedentes han determinado que la OMS haya recomendado que los países adopten políticas al respecto, orientadas a la detección temprana de personas con episodios de psicosis.
