Sumario:Objective: To analyze perception of users with chronic pathologies regarding continuity of clinical and information management among levels in two subnetworks in the Municipal Health Network of Rosario. Methods: Qualitative descriptive study. The study area is formed by the Northeast and South subnetworks. A theoretical sampling was defined in each subnetwork, choosing chronic patients who had used care levels 1 or 2 in the previous six months. Results: Good perception of continuity of clinical management is observed in collaboration/communication among professionals and appropriate follow up. Most problems are related to accessibility among levels. Information continuity is assessed in a positive way; it is perceived that professionals know and use mechanism to transfer information. Conclusion: There is perception of care continuity. Difficulties related to accessibility among levels become the main problem and negatively affect the continuity of clinical management in a general way.
