screening method of ocular pathology in kindergarten and school students at three institutions of Zonal 12 in Montevideo (March-December 2010). Method: Medical students were trai ned with a blended-learning approach (attending classes and on line classes) and offi ce practices. Screening days were double blind and kids were evaluated by a student and an ophthalmologist. Visual acuity with Snellen Chart and ocular alignment using the Hirsch berg test were systematically explored. Results: One hundred ninety-six children between 5 and 8 years old were studied; 13.7 % required referral due to defi cit in visual acuity and 2.5%
because of strabism us. The agreement between students and technicians regarding referral due to visual acuity defi cit was 94% and strabismus 96%. Kappa value for visual acuity was 0.77 and for strabismus 0.53. Students ́sensitivity for visual acuity was 88 .9% and 100% for strabismus. Specifi city was 94.7% for visual acuity and 95.8% for strabismu s. Positive predictive value was 72.7% for visual acuity and 38.4% for strabismus and the negative predictive value was 98.1% for visual acuity and 100% for strabismus. conclusions: The percentage of derivatives is similar to that of previous works. From the results of the study, it would be appropriate to include the screening of visual alterations by trained medical students at educational institutions, since it optimizes interventions at primary health care. |

Introducción. El “Programa Nacional de Salud Ocular” tiene por objetivo abordar las causas evitables o tratables de ceguera. En este marco, el Departamento de Medicina Familiar y Comunitaria ha desarrollado actividades de Promoción de salud y Prevención de enfermedades y problemas oculares en la infancia. Esta actividad se planifico y se llevo a cabo en interdisciplina con la participación de estudiantes de grado de medicina, estudiantes de la licenciatura de oftalmología, licenciados en oftalmología y oftalmólogos, y en intersectorialidad con la participación del Hospital de Ojos, Ministerio de Desarrollo Social y Cámara de Ópticos. Este trabajo incorpora la evaluación de las competencias adquiridas por los estudiantes en la realización del cribado de alteraciones visuales (grado de concordancia inter-observador). Objetivo. Evaluar validez y seguridad de la aplicación de un método de cribado de patología ocular en escolares y preescolares por estudiantes de medicina en tres instituciones del Zonal 12 de Montevideo (marzo-diciembre 2010). Metodología. Se capacitó a los estudiantes de Medicina con una modalidad blended-learning (combinando clases presenciales con la fase on line) e instancias prácticas en consultorio. Las jornadas de cribado fueron doble ciego siendo los niños evaluados por un estudiante y un licenciado en oftalmología. Se exploró sistemáticamente agudeza visual mediante cartilla de Snellen y alineación ocular por test de Hirschberg. Resultados. Se estudiaron 196 niños de 5 a 8 años. Requirieron derivación 13.7% por déficit en la agudeza visual y 2.5% por estrabismo. La concordancia entre estudiante y Técnicos en relación a la derivación por déficit de la agudeza visual fue 94% y por estrabismos 96%. El valor Kappa para agudeza visual fue de 0,77 y para el estrabismo 0,53. La sensibilidad de los estudiantes fue 88.9% para la agudeza visual y de 100% para el estrabismo. La especificidad fue de 94.7 % para agudeza visual y 95.8 % para estrabismo. El valor predictivo positivo fue de 72,7 % para la agudeza visual y 38.4 % para estrabismo y el valor predictivo negativo fue de 98.1 % para agudeza visual y 100 % para estrabismo. Conclusiones. El porcentaje de derivados es similar a trabajos realizados anteriormente. A partir de los resultados del trabajo sería oportuno incorporar la realización del cribado de alteraciones visuales por estudiantes de medicina capacitados, en las instituciones educativas, lo cual optimiza las intervenciones en el primer nivel de atención.. . Neste contexto, o Departamento de Medicina Familiar e Comunitária levou a cabo atividades de Promoção da saúde e Prevenção de doenças e problemas oculares na infância. Este programa foi planejado e realiz ado de maneira interdisciplinar com a participação de estudantes de medicina, de estudantes do bacha relado em oftalmologia, bacharéis em oftalmologia e oftalmologistas, e em parceria coma participação do Hospital de Ojos, o Ministério do Desenvolvimento Social e a Câmarade Ópticos. Este trabalho inclui a avaliação das competências adquiridas pelos estudantes na realização da triagem dos distúrbios visuais (grau de concordância inter-observador). O objetivo foi avaliar a validade e segurança de aplicação de um método de triagempara doenças oculares nos escolares e pré-escolares por estudantes de medicina em três instituições do Zonal Montevidéu 12 (de março a dezembro de 2010). Metodologia: estudantes de medicina foram treinados com um modo blended-learning (combinando aulas presenciais com a fase on-line) e casos práticos em consultório. As jornadas de triagem foram duplo-cegos e as crianças foram avaliadas por um estudante e um bacharel em of talmologia. A acuidade visual foi sistematicamente avaliada usando o gabarito de Snellen e o alinhamento ocular através do teste de Hirschberg. Resultados: 196 cria nças, de 5 a 8 anos, foram estudadas. 13,7% precisaram de derivação por défi cit em acuidade vi sual e 2,5% em estrabismo. A concordância entre estudante e Técnicos relativa ao défi cit de derivação na acuidade visual foi de 94% e 96% em estrabismo. O valor Kappa para acuidade visual foi de 0,77 e de 0,53 para o estrabismo. A sensibilidade dos estudantes foi de 88,9% para acuidade visual e 100% para estrabismo. A especifi cidade foi de 94,7% para acuidade visual e de 95,8% para estrabismo. O valor preditivo positivo foi de 72,7% para acuidade visual e de 38,4% para estrabismo e o valor preditivo negativo foi de 98,1% para acuidade visual e de 100% para estrabismo. Conclusões: A porcentagem de derivados é semelhante a trabalhos realizados previamente. A partir dos resultados do trabalho a cha-se conveniente integrar a realização de triagem de distúrbios visuais por estudantes de medicina treinados em instituições educacionais, otimizando-se dessa maneira as inter-venções nos cuidados primários.
