Early diagnosis, appropriate approach and ordering of imaging tests prevent secondary complications, such as local infection and progression to deep cervical and thoracic fascial spaces. Thus, the present clinical case reports the transoperative occurrence of subcutaneous emphysema during extraction of the lower third molar, its indicated treatment and how we can avoid this type of complication. El enfisema subcutáneo se describe como una complicación infrecuente resultante del paso de aire comprimido a los planos fasciales y se asocia, en odontología, a tratamientos restauradores, endodónticos, cirugía periodontal y extracciones, esta última con una mayor incidencia. El diagnóstico precoz, el abordaje adecuado y la ordenación de las pruebas de imagen previenen complicaciones secundarias, como la infección local y la progresión a los espacios fasciales cervicales y torácicos profundos.
