The NUTRIC score (with IL-6) and its variants NUTRICm (without IL-6) and NUTRICpcr (CRP instead of IL-6) were proposed to evaluate nutritional risk (RN) and even prognosis in critically ill patients.
Methods: Scores with inflammatory biomarkers (NUTRIC and NUTRICpcr) and without them (NUTRICm) were evaluated through a prospective and observational study to discriminate mortality and assess NB in patients ventilated for COVID-19.
Results: 418 patients were analyzed. The median (IQR) age and APACHE II were: 54 years (6-61) and 13 points (10-19). 79% died in the ICU. The highest agreement was between NUTRICm and NUTRICpcr (Kappa= 0.88). The AUCROC (95% CI) to discriminate mortality was: NUTRIC 0.661 (0.598-0.724), NUTRICpcr 0.635 (0.572-0.697) and NUTRICm 0.659 (0.596-0.722). The high RN classification showed differences between the scores with and without an inflammatory biomarker, being significant between NUTRIC and NUTRICm (14.6% vs 22.7%, p 0.0033).
Conclusion: In ventilated COVID-19 patients, the three nutritional risk scores studied discriminate mortality in a similar way, but the assessment of high NR differs between them, with a higher prevalence for the NUTRICm, which omits the inflammatory biomarker. |

El escore NUTRIC (con IL-6) y sus variantes NUTRICm (sin IL-6) y NUTRICpcr (PCR en lugar de IL-6) fueron propuestos para evaluar el riesgo nutricional (RN) e incluso el pronóstico en pacientes críticos. Métodos: Se evaluó mediante un estudio prospectivo y observacional, los escores con biomarcadores inflamatorios (NUTRIC y NUTRICpcr) y sin ellos (NUTRICm) para discriminar mortalidad y valorar el RN en pacientes ventilados por COVID-19. Resultados: Se analizaron 418 pacientes. La mediana (RIQ) de edad y APACHE II fue: 54 años (6-61) y 13 puntos (10-19). El 79% falleció en UCI. La concordancia más elevada fue entre NUTRICm y NUTRICpcr (Kappa= 0,88). El AUCROC (IC95%) para discriminar mortalidad fue: NUTRIC 0.661 (0.598-0.724), NUTRICpcr 0.635 (0.572-0.697) y NUTRICm 0.659 (0.596-0.722). La clasificación de RN alto mostro diferencias entre los escores con y sin biomarcador inflamatorio, siendo significativa entre NUTRIC y NUTRICm (14,6% vs 22,7%, p 0,0033). Conclusión: En pacientes COVID-19 ventilados, los tres escores de riesgo nutricional estudiados discriminan la mortalidad en forma similar pero la valoración del RN alto difiere entre ellos, con una mayor prevalencia para el NUTRICm, que omite el biomarcador inflamatorio. O escore NUTRIC (com IL-6) e suas variantes NUTRICm (sem IL-6) e NUTRICpcr (PCR em vez de IL-6) foram propostos para avaliar o risco nutricional (RN) e até mesmo o prognóstico em pacientes críticos. Métodos: Os escores com biomarcadores inflamatórios (NUTRIC e NUTRICpcr) e sem eles (NUTRICm) foram avaliados por meio de estudo prospectivo e observacional para discriminar mortalidade e avaliar RN em pacientes ventilados por COVID-19.
