A multi-group study (n = 26 departments in Cordoba) was conducted using secondary data sources (provincial statistics, National System of Health Surveillance, 2010 Census). MRABA, MRC, and Case Fatality were developed for 2020 to 2022 and compared based on selected indicators (population with at least one unmet basic need – UBN –, access to sewers, natural gas network, population with no health care coverage or vaccination against COVID-19). The comparison of the rates in the periods was made using the Kruskall-Wallis test and the correlation between the rates every year and the indicators selected was carried out using the Spearman or Pearson method (95% CL), and using Stata v. 17.0 software. Case fatality was greater in 2020 (2.82%; p < 0.01), while MRC was greater in 2021 (1,53‰; p < 0,01); MRABA in 2021 was greater than in 2020 (6,49‰; p = 0,02). Vaccination coverage for the primary scheme was 87.7% in 2021 and 91.4% in 2022, while for the booster shot, the figures were 14.9% and 58.3% respectively. In 2020, case fatality correlated positively with worse socioeconomic conditions; meanwhile, MRC in 2020 and 2021 correlated with better socioeconomic conditions, whereas in 2022 a positive correlation, though not significant, correlated with lower vaccination coverage and worse socioeconomic conditions. Different COVID-19 mortality profiles were found according to the stage of the pandemic; at the beginning of the pandemic, socioeconomic vulnerability was associated with greater case fatality but not higher mortality. The decrease in case fatality and MRC may be the result of high vaccination coverage, active immunity, established treatment, and the prevention measures adopted by the population.

Existe evidencia acerca de la mayor carga de mortalidad por Covid-19 que enfrentan los grupos socioeconómicamente vulnerables; sin embargo algunas investigaciones encontraron que la pobreza se asoció a mayor letalidad, pero no a mayor mortalidad. Nuestro objetivo fue analizar las tendencias en la tasa de mortalidad ajustada por edad (TMAPE) y mortalidad y letalidad por Covid-19 (TMC y Letalidad) según indicadores socioeconómicos en la provincia de Córdoba durante 2020-2022. Se condujo un estudio ecológico multigrupal (n=26 departamentos de Córdoba), empleando fuentes secundarias de datos (Estadísticas provinciales, Sistema Nacional de Vigilancia en Salud, Censo 2010). Se construyeron tasas de TMAPE, TMC y Letalidad para los años 2020 a 2022 y se compararon según indicadores seleccionados (población con al menos una necesidad básica insatisfecha-NBI, acceso a cloacas, gas de red, población sin cobertura de obra social y cobertura de vacunación contra Covid-19).  La comparación de las tasas en los tres períodos se realizó mediante prueba de Kruskall-Wallis y el análisis de correlación entre las tasas en cada año y los indicadores seleccionados se efectuó mediante método de Spearman o Pearson (NC 95%), utilizando software Stata v. 17.0. La Letalidad fue mayor en 2020 (2,82%; p<0,01), en tanto que la TMC lo fue durante el año 2021 (1,53%0; p<0,01); la TMAPE en 2021 fue mayor a 2020 (6,49%0; p=0,02). La cobertura de vacunación para esquema primario fue 87,7% en 2021 y 91,4% en 2022; en tanto que para refuerzo de 14,9% y 58,3% respectivamente. La letalidad se correlacionó positivamente en 2020 a peores condiciones socioeconómicas; en tanto que la TMC en 2020 y 2021 con mejores condiciones socioeconómicas, mientras que en 2022 se encontró correlación positiva, aunque no significativa, con menor cobertura de vacunación y peores condiciones socioeconómicas. Se encontraron diferentes perfiles de mortalidad por Covid-19 según etapa de la pandemia; al comienzo de la pandemia, la vulnerabilidad socioeconómica se asoció a mayor letalidad, pero no a mayor mortalidad. La disminución de la letalidad y TMC podría deberse a la alta cobertura de vacunación, a la inmunidad activa, el tratamiento instaurado y las medidas de prevención adoptadas por la población.
