It is essential to have health surveillance tools capable of comprehensively capturing the complexity and multidimensionality of pesticide exposure. The aim is to validate a Social-Health Vulnerability Scale (SVS) adapted to the Green Belt of the City of Córdoba (CGB), in order to have a robust and reliable tool for conducting health surveillance in horticultural workers and their families. An observational cross-sectional study was conducted (n=89 horticulturists from CGB). Living conditions, health status, work-related factors, and pesticide exposure during 2019-2020 were assessed. These assessments facilitated the design of an SVS consisting of the following dimensions (D): D1 occupational pesticide exposure (use of personal protective equipment), D2 health tracer events (presence of self-reported symptoms, diseases, and accidents), D3 housing conditions (availability of basic services: water, electricity, drainage, sanitation), and D4 horticultural production material conditions (land tenure, type of agricultural machinery, and tools). The construct validity was determined by Cronbach's Alpha coefficient (α=0.72). The internal validity of the EVS was assessed using confirmatory factor analysis (specification, estimation, and evaluation of a structural equation model). The model's plausibility was evaluated through fit indices (χ² square; CFI and TLI >0.90; SRMR <0.09; RMSEA <0.05). Stata 16 software was employed for analysis. The factorial model subjected to verification showed an adequate fit to the data: χ²; CFI=0.955; TLI=0.942; SRMR=0.079; RMSEA=0.046 (0.083-0.536). It consisted of three dimensions: D1, D2, and D3. Covariance between variables in D1 (0.306) and D3 (0.518) parameters improved the model fit. The SVS can be considered a reliable tool for health surveillance in CGB, promoting comprehensive public health policies that consider the multidimensionality of the horticultural context.

La actividad hortícola se realiza en contextos que generan vulnerabilidad, por la exposición continua a plaguicidas producto de las prácticas productivas imperantes, la informalidad laboral y precarias condiciones de vivienda y producción, entre otras. Resulta importante contar con herramientas de vigilancia de la salud capaces de captar la complejidad y multidimensionalidad del fenómeno de exposición a plaguicidas de manera integral. Se propone validar una Escala de Vulnerabilidad Sociosanitaria (EVS) adaptada el Cinturón Verde de la Ciudad de Córdoba (CVCC) con fines de contar con una herramienta sólida y confiable para realizar vigilancia de la salud en trabajadoras/es hortícolas y sus familias.   Se llevó a cabo un estudio observacional de corte transversal (n=89 horticultores del CVCC); se relevaron condiciones de vida, salud, trabajo y exposición a plaguicidas (2019-2020) que permitieron diseñar una EVS constituida por las siguientes dimensiones (D): D1 exposición ocupacional a plaguicidas (uso de equipo de protección personal); D2 eventos de salud trazadores (presencia de síntomas, enfermedades y accidentes auto reportados); D3 condiciones habitacionales (disponibilidad de servicios básicos: agua, electricidad, desagüe, sanitarios); D4 condiciones materiales de producción hortícola (tenencia de tierra, tipo de maquinaria y herramientas agrícolas) con una validez de constructo determinada por el coeficiente Alpha de Cronbach (α=0,72). La validez interna de la EVS se estudió mediante  análisis factorial confirmatorio (especificación, estimación y evaluación de un modelo de ecuaciones estructurales) y la plausibilidad del modelo se evaluó mediante índices de ajuste (X² cuadrado; CFI y TLI >0,90; SRMR <0,09; RMSEA <0,05). Se utilizó el software Stata 16. El modelo factorial sometido a verificación aportó un adecuado ajuste a los datos: X2; CFI=0,955; TLI= 0,942; SRMR=0,079; RMSEA= 0,046 (0,083-0,536). Se conformó por tres dimensiones D1, D2 y D3. También se observó un covarianza entre variables de D1 (0,306) y D3(0,518) parámetros que mejoran el ajuste del modelo. La EVS puede ser considerada una herramienta confiable para realizar vigilancia de la salud en el CVCC y promover políticas de salud pública integrales que contemplen la multidimensionalidad del contexto hortícola.  .
