Population-based analytical-observational study on n=4,924 women aged 50-70 years (target population for mammography practice) who participated in the 2018 National Survey of Risk Factors in Argentina (multistage probabilistic sampling). Indicators of the family environment were considered: a) type of household: single-person, coupled with/without children, single-parent, non-marital multi-person; b) female head of household (yes/no); c) family size (number of members). Significant differences (α=0.05) in the proportion of mammography practice between subgroups were evaluated. By using logistic regression models, association measures (ORs) were estimated between timely mammography (in the last 2 years, yes/no) and the aforementioned variables, controlling for age, income quintiles, and educational level. In the group studied, 63% underwent timely mammography, 64% reported a female head of household, and an average family size of 2.3 (±1.4) members. Although the most frequent type of household was the couple one, the percentage of women in single-parent households with children was double in those who did not undergo mammograms. The percentage of women with timely mammography was significantly higher (p<0.001) in those in coupled households (vs. non-marital), without a large family (vs. families with 4 or more members), and with a male head of household (vs. feminine). Women in coupled households have a greater chance of timely mammography practice (OR 1.58, p<0.001), contrary to what was observed regarding belonging to large families (OR 0.61, p<0.001). In addition, women with the highest educational level and income would be more than twice the chances of performing mammography, compared to lower levels (OR 2.56 and 2.32, respectively, p<0.001). Concluding, the type of household and family size are recognized as social determinants of the practice of mammography in women aged 50-70 in Argentina, with a social gradient by the level of education and income.

El cáncer de mama es la primera causa de muerte por tumores malignos en mujeres argentinas. Si bien se reconocen desigualdades sociales en la realización de mamografías, aspectos relacionados al entorno familiar han sido escasamente estudiados. Objetivo: Identificar determinantes sociales de la realización de mamografías en mujeres de Argentina (2018), con foco en el entorno familiar. Estudio de base poblacional, analítico-observacional, sobre n=4.924 mujeres de 50-70 años (población objetivo para práctica mamográfica) que participaron de la Encuesta Nacional de Factores de Riesgo 2018 de Argentina (muestreo probabilístico multietápico). Se consideraron indicadores del entorno familiar: a) tipo de hogar: unipersonal, conyugal con/sin hijos/as, monoparental, multipersonal no conyugal; b) jefatura de hogar femenina (si/no); c) tamaño familiar (cantidad de miembros). Se evaluaron diferencias significativas (α=0,05) en la proporción de realización de mamografías entre subgrupos. Se estimaron medidas de asociación (ORs) entre la realización de mamografía oportuna (en los últimos 2 años, si/no) y las variables mencionadas, ajustando por edad, quintiles de ingresos y nivel educativo, usando modelos de regresión logística. En el grupo estudiado, 63% realizaron mamografía oportuna, 64% reportó jefatura de hogar femenina y tamaño familiar promedio de 2,3 (±1,4) miembros. Aunque el tipo de hogar más frecuente fue el conyugal, el porcentaje de mujeres en hogares monoparentales con hijos/as fue el doble en quienes no realizaron mamografías. El porcentaje de mujeres con mamografía oportuna fue significativamente mayor (p<0,001) en aquellas en hogares conyugales (vs. no conyugales), sin familia numerosa (vs. familias de 4 o más miembros), y con jefatura de hogar masculina (vs. femenina). Las mujeres en hogares conyugales tendrían más chance de realización de mamografías oportunas (OR 1.58, p<0,001), contrariamente a lo observado respecto a pertenecer a familias numerosas (OR 0.61, p<0,001). Además, mujeres con el nivel educativo y de ingresos más altos tendrían más del doble de posibilidad de realizarse mamografías, comparados con el nivel inferior (OR 2.56 y 2.32, respectivamente, p<0,001). Concluyendo, se reconoce el tipo de hogar y tamaño familiar como determinantes sociales de la práctica de mamografía en mujeres de 50-70 años de Argentina, existiendo un gradiente social por nivel de instrucción e ingresos. .
