Low Resolution Brain Electromagnetic Tomography (LORETA) is a noninvasive EEG-based neuroimaging technique that provides direct information about neural processes.  OBJECTIVES: To evaluate cerebral cortical activity in episodic and chronic migraineurs compared to healthy controls using eLORETA. METHODS: 1 minute of artifact-free wake resting state electroencephalogram epochs from 25 healthy volunteers and 74 migraineurs (25 ictal, 25 interictal and 24 chronic) were analyzed using eLORETA. Subject-normalized delta (1-3Hz), theta (4-7Hz), alpha (8-12Hz), beta (13-30Hz) and gamma (31-45Hz) band activities were contrasted between groups. In addition, activity from specific regions of interest (data-driven) was extracted to for statistical comparison. RESULTS: Differences in resting state activity were consistently found in the subcallosal (BA25), parahippocampal (BA28, BA34, BA35, BA36) and precuneus (BA7) regions. A multivariate statistical comparison revealed significant differences for the group-region-frequency band interaction. CONCLUSIONS: Electrophysiological features of resting state cortical activity differ between migraineurs and healthy controls.  Differences are localized in a handful of regions, some of which have been previously related to the condition using other methods in the past. In comparison with other neuroimaging tools, eLORETA directly assesses cerebral activity and not indirect markers, such as blood flow or metabolism. Furthermore, band-specific findings may provide additional information referring to different levels of neural hierarchy. Additional research is warranted in order to better understand the pathophysiological implications of these findings.

Resumen:  INTRODUCCIÓN: Alteraciones electrofisiológicas ocurren en la actividad cerebral de pacientes con migraña según la fase de ésta. Estos cambios se han visto tanto bajo pruebas de estimulación, como en reposo, bajo distintos métodos. OBJETIVOS: Evaluar diferencias en la actividad cortical de pacientes con migraña episódica y crónica, comparada con controles sanos, y localizarlas. MATERIAL Y MÉTODOS: Segmentos de electroencefalogramas de un minuto, libres de artefactos, durante reposo en vigilia de 25 controles sanos, y 74 pacientes con migraña (25 ictales, 25 interictales, y 24 crónicos) fueron analizados usando exact Low Resolution Electromagnetic Tomography (eLORETA). Los generadores corticales de actividad en las bandas delta (1-3Hz), theta (4-7Hz), alfa (8-12Hz), beta (13-30Hz) y gamma (31-45Hz), normalizados por sujeto, fueron comparados (en todo el cerebro, entre vóxeles) entre los grupos. Luego, actividad de regiones específicas de interés (basadas en la observación), fue extraídas para elaborar modelos estadísticos.  RESULTADOS: Marcadas diferencias en la actividad cerebral en reposo fueron consistentemente observadas las áreas subcallosa (BA25), parahipocampal (BA28, BA34, BA35, BA36) y en la precuña (BA7). Una comparación estadística multivariada demostró significancia para la interacción grupo-región-banda de frecuencia. CONCLUSIONES: Diferencias electrofisiológicas en la actividad cortical en reposo entre pacientes con migraña y sus contrapartes sanas implican a varias regiones, algunas de las cuales han sido previamente ligadas a la enfermedad mediante otros enfoques. En contraste con otras herramientas de neuroimágenes, eLORETA evalúa en forma directa la actividad cerebral, y no marcadores indirectos, como flujo sanguíneo o metabolismo. Además, información específica a las bandas podría proveer pistas valiosas con respecto a diferentes niveles de jerarquía neuronal.
