Negative pressure therapy in complex limb wounds, prior to definitive wound closureIntroductionNegative pressure suction systems are used in the treatment of soft tissue wounds toprogressively reduce their size and depth. By providing a closed and moist environment suitablefor wound healing, it favors the elimination of exudates that can inhibit or delay healing, helpseliminate edema and stimulates granulation. These favorable conditions and evolution of thewound, reduces the incidence of infection.objectiveDescription where negative pressure suction system was used in extensive traumatic injuries tothe extremities.Material and methodsRetrospective, descriptive, observational study, included extensive traumatic injuries of theextremities (upper limb and / or lower limb), treated with the negative aspiration system, prior tothe definitive surgical treatment of the lesion, in the present study they were not taken intoaccount bone injuries (fracture or bone exposure) since the population to be studied did notpresent injuries of this type. The period recorded was 4 years (January 2015 to December 2019).The evolution of the wound with this therapy was observed and described.Results25 cases were analyzed (19 male and 6 female), with lower limb (84%) and upper limb (16%)involvement. The average age is 59 years (range 22 - 71). In 44% (11 patients) received a skin graftas definitive treatment, 24% (6 patients) had a second intention closure after treatment due toreduced wound dimensions, 16% (4 patients) were amputated due to septic complication, beingthese patients who did not respond favorably to the combined antibiotic / vaac therapy andwounds that did not have good irrigation due to the extent of the damage (all supracondylar lowerlimb amputations). Four patients (16%) died from systemic complications.ConclusionsThese injuries usually occur under a traumatic environment of extreme severity, although using anegative suction system is not initially a definitive treatment, we observe a favorable evolutionwith this therapy, being an alternative that conditions a wound for definitive treatment.

Resumen: Terapia de presión negativa en heridas complejas de extremidades, previa al cierre definitivo de la herida Introducción Los sistemas de aspiración por presión negativa, se utilizan en el tratamiento de heridas de partes blandas para reducir progresiva su tamaño y su profundidad. Al proporcionar un ambiente cerrado y húmedo adecuado para la curación de la herida, favorece la eliminación de exudados que pueden inhibir o retrasar la curación, ayuda a eliminar el edema y estimula la granulación. Estas favorables condiciones y evolución de la herida, disminuye la incidencia de infección. Objetivo Descripción donde se uso sistema de aspiración por presión negativa en heridas traumáticas extensas de las extremidades. Material y métodos Estudio retrospectivo, descriptivo, observacional, incluyó heridas traumáticas extensas de las extremidades (miembro superior y/o miembro inferior), tratados con el sistema de aspiración negativa, previa al tratamiento quirúrgico definitivo de la lesión, en el presente estudio no fueron tomadas en cuenta las lesiones oseas (fractura o exposición osea) ya que la población a estudiar no presento lesiones de este tipo. El periodo consignado fue de 4 años (enero de 2015 a diciembre de 2019). Se observo y describio la evolucion de la herida con esta terapeutica. Resultados Se analizaron 25 casos (19 masculinos y 6 femeninos), con afección de miembro inferior (84%) y miembro superior (16%). La edad promedio es 59 años (rango 22 – 71). En el 44 % (11 pacientes) llegaron a un injerto de piel como tratamiento definitivo, 24 % (6 pacientes) tuvieron un cierre por segunda intención posterior al tratamiento debido a unas dimensiones reducidas de la herida, 16 % (4 pacientes) fueron amputados por complicación séptica, siendo estos pacientes que no respondieron favorablemente a la terapia combinada antibiótica/vaac y heridas que no tenían buena irrigación debido a la extensión del daño (todas amputaciones supracondíleas de miembro inferior). Cuatro pacientes (16 %) fallecieron por complicaciones sistémicas. Conclusiones Estas lesiones suelen presentarse bajo un entorno traumatológico de extrema gravedad, aunque utilizar un sistema de aspiración negativa no sea inicialmente un tratamiento definitivo, observamos una evolución favorable con esta terapéutica, siendo una alternativa que acondiciona una herida para el tratamiento definitivo. .
