Within the non-surgical techniques of the saphenous magna the treatment with FOAM, is the practice of election today. This eco-guided sclerotherapy technique allows by endoluminal application of a detergent substance in the form of foam the denaturation of the endothelium, vasospasm and subsequent fibrosis of the vessel. The objective of this method is the treatment of the insufficiency of the saphenous magna, being the advantage of the method, the simplicity of its execution. It does not need to be performed inside an operating room, does not require any type of anesthesia and can be performed inside the clinic.
This work was carried out in the service of Flebologia and Linfologia of the Hospital Nacional de Clinicas - FCM. Applied this treatment in the saphenous magna by foam in 21 patients, 13 females and 8 males, was used sclerosant idroxipolietoxidodecano to 1 % in the form of foam and the ultrasound scanner used was a Terason 3200 T. The technique used was 3 applications with a total 10 cm of foam in the region of saphenous magna. Permanent elastocompression for 96 hours.
In the 80.9% of cases (17) was obtain a obliteration of saphenous magna after treatment, in the remaining 20.1% (4) required a second application to complete the result. Among the expected adverse reactions, pain and local phlogosis, were the most frequent and tolerable; phlebothrombosis occurred in 5% of cases (1), where local thrombectomia with good post-treatment evolution was required.
The treatment with FOAM of saphenous magna is currently the technique of choice, for the good results obtained and for the possibility of performing it outpatiently. To date the results achieved in the service have allowed to solve the problem in the treatment of venous ulcers and tax venous gulfs of the saphenous magna in specifically selected patients. It is part of our project to continue with this methodology for which it has presented itself in the Training and Teaching Committee of the Hospital Nacional de Clinicas for consideration. |

Dentro de las técnicas no quirúrgicas de la safena magna el tratamiento con FOAM, es la práctica de elección en la actualidad. Esta técnica de escleroterapia ecoguiada permite mediante la aplicación endoluminal de una sustancia detergente en forma de espuma la desnaturalización del endotelio, el vasoespasmo y la posterior fibrosis del vaso. El objetivo de este método es el tratamiento de la insuficiencia de la safena magna, siendo la ventaja del método, la simplicidad de su ejecución. No necesita ser realizada dentro de un quirófano, no requiere ningún tipo de anestesia y puede ser realizado dentro del consultorio. Se realizó este trabajo en el servicio de Flebologia y linfologia del Hospital Nacional de Clínicas – FCM. Se aplicó este tratamiento en la safena magna por FOAM en 21 pacientes , 13 del sexo femenino y 8 del sexo masculino, se utilizó esclerosante hidroxipolietoxidodecano al 1 % en forma de espuma y el ecógrafo utilizado fue un Terason 3200 T. La técnica utilizada fue de 3 aplicaciones con un total 10 cm de espuma en la región de safena magna. Elastocompresión permanente por 96 hs. Se obtuvo en el 80,9% de los casos (17) obliteración de safena magna después tratamiento, en el 20,1 % restante (4) requirió de una segunda aplicación para completar el resultado. Dentro de las reacciones adversas esperadas, dolor y flogosis local, fueron las más frecuentes y tolerables; la flebotrombosis se presento en el 5% de los casos (1), donde se requirió trombectomia local con buena evolución post tratamiento. El tratamiento con FOAM de safena magna es en la actualidad la técnica elección, por los buenos resultados obtenidos y por la posibilidad de realizarlo ambulatoriamente. A la fecha los resultados alcanzados en el servicio han permitido resolver la problemática en el tratamiento de ulceras venosas y golfos venosos tributarias de la safena magna en pacientes seleccionados especificamente. Es parte de nuestro proyecto continuar con esta metodologia para lo cual se ha presentado el mismo en el Comite de capacitacion y Docencia del Hospital Nacional de Clinicas para su consideracion.
