A review was performed of medical records of all patients admitted at our Hospital´s intensive care unit having HELLP syndrome during pregnancy or puerperium between February 1999 and February 2010. Patients complicated with hematoma or hepatic rupture were included. Results: Three cases were identified. The average maternal age was 35 years old and the average gestational age at delivery was 33 weeks. The most frequent signs and symptoms were sudden onset of abdominal pain and shock. A distinctive laboratory finding was anemia. Surgical treatment was omental patching in one of the patients and it was conservative with drainages in the other two cases. Maternal and perinatal mortality were 33 %.Conclusion: Based on our experience and the literature review, we can conclude that hepatic hematoma and rupture associated with HELLP syndrome represent a life-threatening complication in which early detection is very important to achieve a proper diagnosis and treatment. Patients should be treated by a multidisciplinary team in reference centers being carefully followed with adequate hemodynamic support and regular images to improve the survival maternal rate.  Objetivos: Describir nuestra experiencia en pacientes que presentaron Síndrome HELLP complicado con hematoma o rotura hepática. Material y métodos: Estudio descriptivo, transversal y retrospectivo. Se revisaron las historias clínicas de pacientes que ingresaron a Terapia Intensiva de nuestro hospital, con diagnóstico de síndrome HELLP durante el embarazo y/o puerperio, en el período comprendido entre febrero de 1999 y febrero de 2010. De ellas se seleccionaron las que presentaron hematoma o ruptura hepática (REH). Resultados: Se identificaron 3 casos de síndrome HELLP con hematoma y/o ruptura hepática. La edad promedio materna fue 35 años; la edad gestacional media al momento del parto fue 33 semanas. Los síntomas y signos más frecuentes fueron dolor abdominal de inicio súbito y shock Hipovolémico. Como hallazgo distintivo de laboratorio se detectó anemia. El tratamiento quirúrgico fue la realización de packing en una paciente y la colocación de drenajes con conducta expectante en las otras. La mortalidad materna y perinatal fue del 33%.Conclusión: Basados en nuestra experiencia y la bibliografía consultada, podemos concluir que el hematoma y la ruptura hepática asociados a síndrome HELLP constituyen una complicación grave donde el reconocimiento temprano es la base para un diagnóstico y tratamiento oportuno. El abordaje debe ser multidisciplinario, en centros de alta complejidad, con adecuado soporte hemodinámico y seguimiento imagenológico para mejorar la sobrevida de estas pacientes Universidad Nacional Córdoba.
