The aim was to analyze the causes which motivate a prolonged psychiatric hospitalization of children/adolescents in the city of Cordoba. Exploratory and retrospective study, mixed technique, qualitative and quantitative study based on clinical records of children/adolescents hospitalized in Hospital de Niños de Cordoba between years 2009-2016 with hospitalizations of more than 30 days. The cases were divided into two groups, group with prolonged hospitalizations from 31 to 59 days (GIP) and group with prolonged hospitalizations of more than 60 days (GIMP). The data were analyzed by frequency analysis, chi-square test and qualitative analysis of documentary records. Five hundred fifty-seven hospitalizations were performed in the period. In 15.9% (n = 89) of the registries was observed more than 30 days of hospitalization, in which 64 corresponded to hospitalizations between 30-60 days (GIP) and 25 hospitalizations over than 60 days (GIMP). The most frequent reasons of hospitalization for the GIP were suicidal behaviors 23.44%, and psychomotor excitement episodes 15,6%. In the GIMP were episodes of psychomotor exacerbation 24% and hetero-aggression 32%. Reasons of intake, as well as diagnosis of release, showed a differentiated behavior by gender. In males, the most frequent diagnosis was behavior disorders and in females developmental personality disorders. In the GIMP the 36% (n=9) were children /adolescents who lived in conditions of shelter, being that group the one that presented the highest risk of re-hospitalization (p <0.01; OR = 3.6). Regarding health, the qualitative analysis showed difficulties in access to intermediate devices (day hospital), stigmatizing behavior attitudes in healthcare professionals and situations of violation of the rights of children living in institutional care both from their families and from State institutions. Prolonged hospitalizations evidenced a differentiated behavior by gender associated to pathologies of impulsive-aggressive spectrum and factors of social vulnerability.

La internación en salud mental es un dispositivo de atención de tercera complejidad y el último recurso terapéutico a utilizarse, por el menor tiempo posible, cuando otras instancias terapéuticas han sido agotadas  (Ley de Salud Mental N°26.657). El objetivo fue analizar las causas que motivan una internación psiquiátrica prolongada en niños-adolescentes de la ciudad de Córdoba. SE realizó un estudio exploratorio, retrospectivo, técnica mixta, en base a registros clínicos de niños/adolescentes hospitalizados en el Hospital de Niños de Córdoba entre los años 2009- 2016 con internaciones de más 30 días. Los casos fueron 89 y se dividieron en: grupo con internaciones prolongadas de 31 a 59 días  (GIP) y grupo con internaciones prolongadas de más de 60 días (GIMP). Los datos cuantitativos se analizaron por análisis de frecuencias, test de chi-cuadrado. El análisis cualitativo se realizó por medio de análisis de contenido de registros documentales e informes técnicos (categorías de análisis salud/derecho).   Se efectuaron 557 internaciones en el periodo y el 15,9% (n=89) presentó más de 30 días de internación. De estos,  64 internaciones en el GIP   y 25 internaciones  en el GIMP. Los motivos de internación más frecuentes para el GIP fueron los comportamientos suicidas 23,44% y episodios de excitación psicomotriz 15,63%. En el GIMP fueron los episodios de excitación psicomotriz 24 % y  hetero-agresión 32%. Tanto los motivos de ingreso como los diagnósticos de egreso mostraron un comportamiento diferenciado por género. En los hombres los diagnósticos más frecuentes fueron los trastornos del comportamiento y en mujeres los trastornos del desarrollo de la personalidad. EL 36% (n=9) del GIMP eran niños/adolescentes que viven en condición de hogarizados siendo ese grupo el que presentó mayor riesgo de re-hospitalización (p<0,01; OR=3,6).  Respecto a salud, el análisis cualitativo mostró dificultades en el acceso a dispositivos intermedios (hospital de día), actitudes de comportamiento estigmatizantes en los profesionales de la salud y situaciones de vulneración de derechos de los niños/adolescentes hogarizados tanto de parte de las familias como de las instituciones del Estado.  Las hospitalizaciones prolongadas mostraron un comportamiento diferenciado por género, se asociaron a patologías del espectro impulsivo-agresivo y  factores de vulnerabilidad social.
