| Sumario: | The SARS-CoV-2 virus, responsible for the COVID-19 pandemic, is the causal agent of acute respiratory disease. Immunocompromised persons may develop more severe respiratory symptoms and cases of persistent infections have been reported. The objective of this work was to describe 3 cases of persistent SARS-CoV-2 infections in immunosuppressed individuals from Córdoba.
Case 1: HIV+ patient, male, 50 years old, positive for SARS-CoV-2 by real-time RT-PCR in an oropharyngeal swab sample (HOF) on 12/11/2021 (Ct=29.3), presenting respiratory symptoms. Repeated samples taken on 13/12/2021, 28/12/2021 and 7/1/2022 were positive, with Cts of 23.6, 23.2 and 28.6, respectively. The complete viral genome was sequenced in the sample from 13/12, and presented an infrequent mutation profile; the lineage could not be determined. No further clinical information could be obtained.
Case 2: patient with Lupus, female, 47 years old, positive for SARS-CoV-2 by real-time RT-PCR in HOF on 27/12/2021 (Ct=29.2), when she presented dyspnea, headache, odynophagia, cough, anosmia and rhinitis. The typing by Real-time RT-PCR indicated infection by the variant of interest (VOI) Lambda. Subsequent detections were positive: 19/1/2022 (Ct=22), 24/1/2022 (Ct=25.8), 29/1/2022 (Ct=23.5), 4/2/2022 (Ct =23.7). The sample from 19/1 was re-typed and the complete genome was sequenced, resulting in VOI Lambda by both techniques. Furthermore, this sample was isolated in Vero cl76 cells, confirming viral infectivity. No further clinical information could be obtained.
Case 3: HIV+ patient, female, 32 years old. Positive for SARS-CoV-2 by real-time RT-PCR in HOF on 5/2/2022 (Ct=24.3), when fever, headache, cough, sore throat, and dyspnea were present, and in subsequent samples: 17/2/2022 (Ct=20.7), 24/2/2022 (Ct=19.1), 14/4/2022 (Ct=25), 20/4/2022 (Ct=20.6) and 27/4/2022 (Ct=21.6). Complete genome sequences were obtained in samples from 24/2 and 20/4, resulting in the variant of concern (VOC) Omicron, lineage BA.1.15. Until 20/4, the patient had fever, cough, myalgia, diarrhea, vomiting, and abdominal pain.
This work describes, for the first time, persistent SARS-CoV-2 infections in immunosuppressed patients from Córdoba, in accordance with reports from other countries, alerting the health team about their occurrence to perform an active clinical monitoring. This population could constitute a reservoir for the emergence of new viral variants. |
