Substantial progress has been made in the characterization of molecular abnormalities in the EGFR gene, ALK gene fusion andPDL-1 expression in NSCLC, which are being used as molecular targets and predictive biomarkers.The purpose of this study was to determine the frequency of molecular alterations in the EGFR gene, ALK gene fusion and PD-L1 expression and toevaluate the association with age, gender and tobacco habit in the local population.We studied 171 biopsies and resection samples from patients (pts) with NSCLC during the period 2014 - 2019. Characteristics of histopathological type,immunohistochemical (IHC) and molecular profile were analyzed, a subgroup of 39 pts was analyzed for PD-L1 expression. Molecular profiles werecorrelated with different clinical variables (age, sex and tobacco use). Qualitative variables were compared with the chi-square or Fisher test andquantitative variables were compared with the Test-T.Tobacco use was associated with sex 29.82% and 59% of smokers were women and men, respectively (p = 0.012). Women were younger than men (p =0.032). EGFR alterations ware associated with sex (p = 0.002), women were more likely to have positive gene alterations. Only 1.8% of subjects reportedALK alterations and they were not related to sex (p = 0.55). In a patients subgroup where PD-L1 was tested, univariate analysis showed no association ofPD-L1 expression with sex (p = 0.3006), age (p = 0.7747), tobacco habit (p = 1) and EGFR gene mutation (p = 1).The results showed a frequency of expression of mutations in EGFR and ALK fusion oncogene comparable to the data reported in the westernpopulation. The alterations present in the EGFR are the most prevalent in women with adenocarcinoma.

Keywords: lungcancer; EGFR; ALK; adenocarcinoma Durante las últimas décadas se han producido cambios en la epidemiología del cáncer de pulmón de células no pequeñas (CPCNP), hubo un incremento de la estirpe histológica adenocarcinoma. Se ha logrado un progreso sustancial en la caracterización de las anomalías moleculares en el gen EGFR, fusión de genes ALK y expresión de PDL-1 en CPCNP, que se están utilizando como dianas moleculares y biomarcadores predictivos. El objetivo del tragajo fue determinar la frecuencia de alteraciones moleculares en el gen del EGFR, fusión de genes ALK y la expresíon de PD-L1 y evaluar asociaciones con edad, género y consumo de tabaco en población local. Se estudiaron 171 biopsias y muestras de resección de pacientes (pts) con CPCNP durante el período 2014 - 2019. Se analizaron las características del tipo histopatológico, inmunohistoquímico (IHC) y el perfil molecular, un subgrupo de 39 pts se analizó la expresión de PD-L1. Los perfiles moleculares se correlacionaron con diferentes variables clínicas (edad, sexo y consumo de tabaco). Las variables cualitativas fueron compardas con el test chi-cuadrado o test de Fisher y las variables cuantitativas fueron comparadas mediante el Test-T. El consumo de tabaco se asoció con el sexo el 29.82% y el 59% de los fumadores eran mujeres y hombres, respectivamente, (p = 0.012). La edad de las mujeres fue menor con respecto a los hombres (p = 0.032). Las alteraciones del EGFR se asociaron con el sexo (p = 0,002), las mujeres tenían más posibilidades de tener alteraciones positivas del gen. Solo el 1.8% de los sujetos informaron alteraciones de ALK y no estaban relacionadas con el sexo (p = 0.55). En subgrupo de paceintes donde PD-L1 fue testeado, el análisis univariado no mostro asociación de la expresion de PD-L1 con el sexo (p = 0.3006), la edad (p = 0.7747), el consumo de tabaco (p = 1) y la presencia de la mutacion del gen EGFR (p = 1). Los resultados mostraron una frecuencia de expresión de mutaciones en EGFR y del oncogén de fusión ALK comparables con los datos reportados en la población occidental.
